Cost is one major impediment since an electronic system’s implementation necessitates a large investment in hardware, software licensing, training expenditures, and ongoing maintenance costs (Schmeelk et al., 2022). Another hurdle is healthcare professionals’ opposition, who may be accustomed to outdated paper-based methods or feel overpowered by new technology. Issues like data entry mistakes or missing records brought on by insufficient training or a lack of uniformity are among the difficulties associated with H&P documentation (Schmeelk et al., 2022). EMRs can sometimes be challenging to use, frustrating healthcare professionals and reducing productivity.

Advanced practice registered nurses (APRNs) have a legal and ethical obligation to correctly and completely record all facets of patient care. Documentation facilitates communication among healthcare professionals, guarantees continuity of care, and serves as proof in court. Medical documentation requirements include ensuring that all patient data is recorded completely, precisely, and on time (Kasaye et al., 2022). All evaluations, treatments, drugs prescribed, vital signs, and any alterations in the patient’s condition must be documented by APRNs. Documentation must be precise and concise to guarantee that other healthcare professionals can quickly grasp the patient’s situation.
Legal Considerations in EMR Documentation for APRNs
Legal considerations of documentation are equally important for APRNs. Documentation is evidence in legal proceedings such as malpractice lawsuits or disciplinary actions against healthcare providers (Kasaye et al., 2022). Therefore, APRNs must document everything they do when providing care to patients. APRNs must also be aware of their state’s laws regarding documentation. Each state has its laws governing what should be documented in medical records and how long they should be kept. Failure to comply with these laws can result in disciplinary action against healthcare providers.
