The fragmented nature of the American healthcare system calls for government officials to consider the implementation of programs to coordinate the care that patients receive. According to Agency for Healthcare Research and Quality (2018), the main purpose of care coordination is to meet the needs and preferences of patients by providing high-quality health care. One of the initiatives in that regard that was endorsed by the government is the Care Coordination Quality Measure for Primary Care (CCQM-PC). As Heil (n.d.) notes, the CCQM-PC’s objective is to fill the gap in the care coordination evaluation by assessing adult patients’ experiences with care coordination in primary care facilities. It was designed for use in healthcare research and appraisal, with potential applications to improve the quality of primary care.
One framework one can use to evaluate how the initiative improves care is the Triple Aim. As per Norman (2020), the Triple Aim was developed by the Institute of Health Improvement, according to which new projects are to be developed simultaneously pursuing the three most important health system dimensions. These are the improvement of the patient experience of care, the improvement of populations’ health, and the reduction in the cost of healthcare per capita. Agency for Healthcare Research and Quality (2018) states that the implementation of the CCQM-PC has the potential to improve patients’ health and care experience as well as reduce the costs of practice. Moreover, it can elevate populations’ health by lowering patient confusion and helping to ensure a single care plan, the approval of which can be coordinated by multiple providers. When it comes to my future career in healthcare, I feel like it is important to join only the organizations for which care coordination is one of the priorities. That way, I will be certain that I work in a place that strives for outstanding results using a righteous approach.
