Therefore, as a caregiver, I would first incorporate multidisciplinary and collaborative techniques to manage their intricate conditions. This entails engaging closely with patients and their families to create customized treatment schedules that encompass all the needs and requirements of the former. Educating patients about self-care, the need for lifestyle adjustments, and providing emotional support and therapy are some of the strategies that I would prioritize at this stage.

Secondly, I would leverage IoMT modalities to help monitor medication uptake, set alerts for future appointments, and track patients’ blood sugar levels. Wearables can be fitted on patients to track their progress in home-based care contexts. Concurrently, related modalities like mobile apps can be deployed by the patient’s family to help them monitor the patient’s prognosis. This is because telehealth has been proven effective in providing reliable medical care for patients in remote settings over the years.

Conclusion

Managing diabetes and kidney disease demands thorough and integrated strategies considering different underlying social, medical, and lifestyle aspects. Therefore, by working closely with patients and their families, I would improve health outcomes for these patients suffering from the aforementioned chronic diseases. The breakdown of treatment options in this paper can be useful to nurses and other healthcare practitioners in determining the appropriate curative modalities that would be convenient for kidney disease and diabetic patients.
