Nursing Care Model Observed

The model of care exhibited in a healthcare department dictates the manner in which medical support is delivered to different clients. Nurse practitioners (NPs) can use various care delivery models to meet the diverse needs of their clients (Mattila et al., 2014). The nursing practices embraced in a pediatrics facility were identified in order to complete this exercise. The family nursing institution was mainly taking care of individuals in need of critical care such as elderly citizens. The primary nursing model was evident in the facility. The NPs and other caregivers were observed to work independently in an attempt to offer quality care. Using this observation, this paper describes various care models that can be used to improve the outcomes of many clients.

Article One

The first article is “Perceptions of a primary nursing care model in a pediatric hematology/oncology unit”. According to the authors, a relationship-based strategy is what makes the primary nursing care model effective in different healthcare settings. After conducting a powerful research study, the scholars observed that many NPs were happy with the advantages associated with the primary care model. The patient-nurse relationship established in the unit supported healthcare delivery. Unfortunately, the use of this model made it hard for nurses to collaborate with their workmates (Nadeau, Pinner, Murphy, & Belderson, 2016). This kind of gap, according to the article, affects the nature and quality of care available to more patients. The authors support the use of teamwork whenever embracing the power of this primary care model. Positive relationships with patients and other nurses can deliver positive results. Such relationships will definitely promote new practices such as problem-solving and critical thinking in the healthcare environment.

Article Two

The article “The effects of primary nursing care model: A systematic review” shows conclusively that the use of the primary nursing care process has the potential to transform the outcomes of many patients. The authors believe strongly that the healthcare delivery model supports the health needs of the targeted patients. RNs embracing the use of the healthcare delivery approach address most of the challenges affecting their performance such as burnout and workplace dissatisfaction (Mattila et al., 2014). The model is associated with positive working environments, personal empowerment, and realization of goals. Unfortunately, the primary nursing care model fails to promote evidence-based practices such as teamwork and collaboration. However, nurses can use the model in an attempt to provide evidence-based and personalized care to their patients.

Article One

The article “Nurse practitioner care model: Meeting the health care challenges with a collaborative team” begins by acknowledging the fact that the modern nursing environment should be characterized by cost-effective and quality medical services. The best approach towards achieving this goal is by redesigning the role of the NP in different healthcare settings. The use of collaborative teams is an approach that can result in operational effectiveness. Individuals who embrace the model can improve the outcomes of the targeted patients and minimize the chances of rehospitalization (Kutzleb et al., 2015). The studied setting indicated that the institution recorded positive financial performance. With the use of the care model guided by team nursing, hospitals can record increased patient satisfaction and realize their potentials. This model should therefore be embraced by healthcare institutions that want to meet the emerging needs of their patients. This is the best approach that can result in operational effectiveness.

Article Two

The article “Exploring new graduate nurse confidence in interpersonal collaboration: A mixed-methods study” examines the importance of interprofessional teams in nursing practice. The absence of teamwork, collaboration, and confidence can result in poor health services (Plaff, Baxter, Jack, & Ploeg, 2014). A team-based approach to healthcare delivery can therefore address these gaps. The authors indicate clearly that team-based nursing attracts more professionals in order to improve performance. Nurses who want to record positive results should always focus on the importance of teamwork. The implementation of a team-based approach is capable of addressing the needs of the targeted clients. The team nursing strategy makes it easier for the nurses to form supportive relationships, promote collaboration, and respect each other (Plaff et al., 2014). New practices such as positive communication and critical thinking emerge in the targeted department. Healthcare professionals and leaders should therefore identify the best interventions in order to promote interprofessional collaboration. By so doing, the nurses will acquire new experiences and eventually become competent providers of sustainable care.

Implementation of the Primary Nursing Care Model

The studied healthcare setting showed conclusively that the primary nursing care model was appropriate and effective. The NM in the department always monitored the performance of the nurses. The nurses were willing to focus on the unique needs of their respective patients. This approach made it easier for the department to become a leading provider of personalized care (Nadeau et al., 2016). Most of the healthcare workers were observed to establish positive relationships with their patients. This kind of relationship makes it easier for nurses to identify the unique challenges affecting different patients. The facility has managed to deliver evidence-based, timely, patient-centered, and adequate nursing care to targeted patients. The nursing care model has continued to promote the concept of cultural competence in the institution. This is the case because every caregiver understands the importance of monitoring and respecting the cultural needs of the patients. Consequently, many patients have received quality medical support from the identified department.

Recommending a Different Model

There are various nursing care models that can be used by healthcare workers to support the changing needs of different patients. The use of the primary nursing care model has presented positive results in the identified department. However, there are other powerful models that can be used to make a difference in the facility. The Transitional Care Model (TCM) is one of these models and focuses on the concept of continuous care (Hirschman, Shaid, McCauley, Pauly, & Naylor, 2015).

This model brings together different healthcare professionals in order to identify the unique problems affecting different patients. The concept of teamwork emerges in order to promote the delivery of evidence-based care. The care management process is also managed in order to maximize patient outcomes. Nurses should be the pioneers of the model and attract other stakeholders. Patients in need of critical care can benefit from this continuous approach to healthcare delivery. The use of TCM in the targeted healthcare setting can make a difference for many stakeholders. The model will make it easier for the facility to acquire and manage healthcare resources. Evidence-based approaches will be considered in order to provide quality and safe care to targeted patients. Hirschman et al. (2015) believe strongly that the model has the potential to transform the organizational culture experienced in the hospital. The targeted patients will be empowered and realize their health goals.

The implementation of this model will encourage different nurses to support new changes that focus on new practices such as teamwork and collaboration. The quality of nursing care will increase because every stakeholder will be part of the healthcare delivery process. Members of the staff will have their challenges addressed and eventually maximize patients’ outcomes (Hirschman et al., 2015). This model is therefore appropriate especially in different hospitals providing care to elderly patients.

Summary: New Lessons from the Exercise

The first lesson gained from this exercise is that nursing institutions can use different models to achieve positive results. Such care delivery approaches can meet the needs of the targeted patients, promote collaboration, and address the problems affecting healthcare workers. However, each of the models has specific weaknesses that can affect patient outcomes. It would therefore be appropriate for NPs to be aware of the unique strengths and weaknesses associated with each healthcare delivery model. The knowledge can guide the workers to use the best models that have the potential to support the diverse needs of the targeted clients. The TCM model appears to be a superior care delivery model because it depicts most of the attributes promoted by the other approaches (Hirschman et al., 2015). Issues such as teamwork, continuous improvement, and provision of evidence-based care are critical whenever identifying the most appropriate model. The other lesson is that healthcare institutions should be ready to implement new changes and adopt different care delivery models depending on the issues affecting performance. By so doing, evidence-based ideas will emerge and eventually meet the needs of more patients with diverse needs (Nadeau et al., 2016). It will also be appropriate to understand each nursing care model can be applied in a specific healthcare environment depending on the targeted needs of the clients.
