To be diagnosed with depression, the episodes must show the symptoms for fourteen consecutive days. The disorder affects an individual’s ability to carry out daily activities, and a decrease in appetite is observed. There is no or minimal interest in accomplishing activities patients found enjoyable before. Symptoms may differ from one patient to another depending on the age and conditions provoking the manifestation of the disease (Schnellbächer & Grözinger, 2020). In the case under study, the mother noted the child’s mood as sad and noticed a loss of appetite. It is usual for children of his age to enjoy the company of other children, but this particular child is withdrawn from the others. Depressed people tend to either over-eat or eat less than usual, and in this case, the boy has a reduced appetite. Chances that depression might have developed during growth are minimal because results from the physical tests were successful. It is essential to examine the child’s physical and mental state to prescribe appropriate drugs and therapies for overcoming the disorder.
Examination of Boy with Depression Symptoms
The observation showed that the child got irritated quickly, meaning that the people around the eight-year-old and the surroundings were unappealing. The child might not have shown any suicidal thoughts, but he is interested in death; this may trigger him to commit suicide. Moreover, since the client thinks about himself as dead, he has completely detached himself from the people around him. He demonstrates the desire to be alone and not feeling anything or having any connection. A decision will be made based on four significant symptoms depicted by the child, including what causes the feeling of sadness and the increase in his agitation over minor issues. Some of his peers might have done or said something to him that made the boy avoid their company. The child should tell the difference between how he feels when he is alone and with his friends. The death issue will be handled last, and the client should tell the therapist why he thinks of himself as dead or why he believes there might be pleasure in death.
Client Treatment
Biomedical therapies will be minimal at the boy’s age because they are only appropriate in extreme cases. Life-threatening conditions include drug addiction or anxiety, which is not the case in the situation under study. Firstly, the child will be administered with Zoloft drug. It is the fittest decision for a child because it has fewer side effects that are treatable when they become severe. Paxil and Wellbutrin have concrete conditions that should be used, making Zoloft the most suitable option for an eight-year-old. Zoloft administration should be combined with Cognitive-behavioral therapy to assist the patient in learning the motives and conduct of other children, which will reduce occasions of getting irritated over minor issues caused by his peers (Paul, 2021). It is also crucial to examine the child’s relations with his friends, as peers may be intentionally mean, making the eight-year-old feel uncomfortable because of possible racial disparities within his community.
CBT teaches how to develop a sense of self-confidence in one’s abilities. The confidence will moderate the feeling of worthlessness that may become oppressive for the patient. This therapy may facilitate the patient in accepting his different skills and assure that features do not make the patient separate from other people. The boy will learn to defend himself properly without getting exasperated or fighting when his friends taunt him. Moreover, undergoing the therapy will solve significant problems successfully before handling other situations. At the end of these first sessions, the boy will be better positioned to deal with irrational conduct caused by emotional stress. It is essential to note that Zoloft may cause a patient to be irritable, talkative, and confused, thus making communication with the client difficult (Binar et al., 2018). Ethical considerations only allow specialists to use the information provided by the client freely. When patients experience mood swings, they may speak rudely to the therapist, and the specialist should consider these issues according to ethical doctrines. The therapist should not act in a way that will harm patients in their vulnerable state.
Paxil 10mg Oral Administration
Paxil is a prescription used to treat early symptoms of depression, and it may be used alone or combined with other drugs. It is also used to reduce post-traumatic experiences anxiety or treat social phobia. The main goal of using Paxil is to ensure symptoms do not prolong as the solution is being sought. Curbing the symptoms at an early stage is the specialness of Paxil compared to the other two options (Tang et al., 2020). The most appropriate therapy session to be used together with Paxil is dialectical behavior therapy. Sometimes depression arises from personal issues, especially when they are beyond a person’s understanding. Inability to handle interpersonal problems may lead one to develop emotional stress that dealing with daily challenges becomes a significant problem. Paxil is administered depending on the client’s medical condition and his response to other treatments before. Instructions are always provided in the leaflet; the patient should understand them before the usage and ask for clarification on anything that appears complex. It is advisable to continue using the drug every day at the same time for several weeks for a positive change to be observed.
DBT helps curb negative thoughts that automatically come when in pain, thus avoiding any impulsive behavior. Whenever the boy finds himself in a situation, he will tolerate issues and accept himself in any condition. It might help the child improve his mindful skills and understand how to solve personal problems within the community. Getting a distraction or self-soothing will help him deal with any misunderstanding and equip him with skills to engage all his senses in any circumstances. At the end of this session, there is an opportunity that the child will focus on essential issues more attentively. This session will also facilitate restoring his appetite because it helps regulate self-destructive behaviors such as starving oneself. Side effects from Paxil are slightly severe compared to those from Zoloft, and that is why its application should be minimal. Ethical considerations do not allow therapists to invade their client’s privacy, but unusual risk talking behavior and blurred vision makes the situation difficult. Paxil causes rational thoughts and mood swings which becomes a barrier when a therapist wants to acquire information.
Wellbutrin 75mg Oral Administration
Wellbutrin antidepressant is used to treat significant disorders in depression and other seasonal diseases. It has diverse uses and can cure more diseases other than those mentioned in the Wellbutrin medical guide. The drug is not appropriate for patients with eating disorders and seizures. It is vital to note that the overdose of Wellbutrin might cause intoxication up to death (Schmit et al., 2017). Wellbutrin is usually administered together with Psychodynamic therapy to help counselors know the unconscious processes depicted in the current behavior. It enables patients to discover how past experiences may influence their character. Psychodynamic therapy aims at changing the aspect of one identity or personality (Butenko, 2021). Being a Black fellow among a white-dominated society may be the reason the boy is reserved from others. No external forces like bullying or abuse have been mentioned, thus, indicating that all the characteristics displayed by the child are mentally activated. Wellbutrin is unique in that it takes a short time to work hence minimizing the recovering period.
At the end of this session, it is anticipated that the boy will change his perception of the ideas he possesses. It will also allow the child to overcome any experience that may have triggered his present behavior. However, the drug may provoke minimal communication due to seizures and convulsions (Ryvlin & Nguyen, 2021). For instance, in being overexcited, a patient will hardly follow instructions on the dosage. Ethical considerations as minimal physical interaction will be considered as that the way to calm seizures and convulsion. Therefore, patients should know the side effects and make decisions on their own before a specialist can administer the drug.
Summary
All the decisions above aim at restoring the normal mental stability of the patient. The combination of Zoloft and CBT will help the child to discover his unique abilities and improve his confidence. Paxil and DBT may facilitate considering the boy’s behavior even before the depression symptoms emerge. It will also provide skills on stabilizing his emotions even when in pain to avoid any instant rational decisions. Wellbutrin and Psychodynamic therapy will make the child disclose his problems and help him to overcome obstacles. Through this session, the boy will understand his past experiences and understand how to differentiate between essential and inconsiderable issues. By undergoing CBT, DBT, and Psychodynamic therapies, the boy will be able to restore his relationships with friends, become emotionally powerful, and improve his ability to control instant emotional instability before making decisions.
