Description and Symptoms
Leukemia is a cancer of the blood and bone marrow characterized by the uncontrolled proliferation of white blood cells (WBCs). The specific mechanisms differ by leukemia type but generally involve genetic alterations in a single bone marrow cell. These changes prompt the cell to replicate swiftly and impede the production of healthy blood cells.
The primary forms of leukemia in kids are acute lymphoblastic leukemia (ALL) and acute myeloid leukemia (AML), with ALL being the most prevalent. Epidemiologically, ALL makes up roughly 75% of leukemia occurrences in children, with the highest frequency among kids aged 2-5 years (Namayandeh et al., 2020). AML is less common but can manifest at any age. Leukemia is the most prevalent cancer in kids and teenagers, with around 3,500 children diagnosed with leukemia annually in the United States.
Clinical observations in a child with leukemia may encompass paleness, weariness, and sluggishness as a result of anemia. Tiny red or purple spots, large bruises, or bleeding may be evident due to a decrease in blood platelets. Fever and susceptibility to infection may arise from a low count of a specific type of white blood cell.
Swelling of the liver, spleen, and lymph nodes can be detected, and there may be discomfort in the bones. Potential alternative diagnoses to consider include a rare type of anemia, glandular fever, other conditions involving bone marrow infiltration, and disorders of abnormal bone marrow cell growth (Tebbi, 2021). Unexplained low blood cell counts, organ enlargement, and the child’s age can help distinguish these conditions from leukemia.
Treatment and Care
The strategy for handling a child with suspected leukemia involves verifying the diagnosis through diagnostic assessments like complete blood count (CBC), peripheral blood smear, and bone marrow biopsy with cytogenetic analysis. If leukemia is confirmed, it’s crucial to categorize it as ALL or AML, as this further impacts the treatment plan (Kaplan, 2019). Treatment generally consists of chemotherapy, divided into three stages: induction, consolidation, and maintenance.
Supportive care, including transfusions, infection management, and nutritional support, is crucial. Follow-up plans are intensive and include regular CBCs, bone marrow evaluations, and monitoring for potential complications of the disease and treatment. Consultation with pediatric oncologists is essential for the treatment of leukemia (Kaplan, 2019). Occasionally, additional experts, such as pediatric neurologists, endocrinologists, or fertility specialists, may be required to address potential treatment ramifications.
